Consent forms
Write consent templates once, then get each patient's signature on screen or on paper, with a full record.
Treatment consent forms record that the patient understood and agreed to a procedure such as an extraction, a root canal or an implant. They're separate from the data-processing consent captured at registration (see Patient consent).
1. Create templates
Clinic Admins and Doctors write templates once in Consent Form Templates → Add Template ("Create reusable consent text for a common procedure."):
- Name, for example Root Canal Consent.
- Description, for example Consent for root canal (endodontic) treatment.
- Consent text: the wording. Use these placeholders, filled in automatically for each patient:
| Placeholder | Becomes |
|---|---|
{{patientName}} | The patient's full name |
{{doctorName}} | The treating doctor |
{{clinicName}} | Your clinic's name |
{{procedureName}} | The procedure |
{{date}} | The date |
Example:
I, {{patientName}}, authorize {{doctorName}} at {{clinicName}} to perform {{procedureName}}.
I have been told about the procedure, the alternatives, the risks (including pain, swelling and
the possibility that further treatment may be needed) and the cost. Signed on {{date}}.
To stop offering a template, open it and click Deactivate. Forms already created from it are not affected. Reactivate brings it back.
2. Create a consent form for a patient
- On the patient's page, open Consent forms → New consent form, pick a Template, and create it.
- In Quick Visit, procedures that usually need consent show Consent recommended, and the saved visit offers the right forms.
- In Start Visit, the Consent step does the same.
The text is frozen when the form is created, so later edits to the template don't change a form a patient already saw.
3. Get it signed
| Option | When | What happens |
|---|---|---|
| Sign | The patient signs on the screen (tablet, phone or touchpad) | The Sign consent form pad captures the signature. Clear to redo |
| Mark signed on paper | The patient signed a printed copy | Record Signed by, Relationship to patient (Self, Parent/Guardian…), Signed on, and optionally upload a Scanned copy |
| Decline | The patient refused to sign | Recorded as declined |
| Void | The form was made by mistake | Marks the record no longer valid. A correction, not a reversal, and can't be undone |
In Quick Visit you can also download the form as a PDF to print.
Statuses
Awaiting Signature · Signed · Signed (Paper) · Declined · Void
For a minor or a patient who can't sign
Choose Mark signed on paper, enter the guardian's name in Signed by, and set Relationship to patient (for example Parent/Guardian).
Where to find them
The patient's page lists all their consent forms. View all consent forms opens the full list with each form's status.
